I bought an app over the weekend. After I entered my username and password, I was prompted to enter the password again. Then I had to answer three security questions, e.g. who was your favorite teacher, Etc. I was then asked to give a rescue email address, which I gave. I received a message from "Apple" on the email account whose address I gave, asking me to click on a link to confirm and then to type in my username and password.
