Hi there, in regards to what you need to have uploaded, check out this link as 
an example on how to configure phorm:

You will need to copy the code of your config file to this forum so I can have 
a look at whats going on. Ensure you are using the correct variable names and 
remember that they are case sensitive. Also check the syntax in the 
documentation and that you are following it closely, without seeing your code, 
thats about all can do...
