In phorm.php look for the following (starts on line ~380)
$ph_Vers = "Phorm v3.0.4";
$ph_HTLink =
"<BR><BR><CENTER><FONT SIZE=1>".
"<A HREF=\"http://www.phorm.com/\";>$ph_Vers</A>".
"</FONT></CENTER>";
$ph_HTTag =
"\n\n\n\n-- \n$ph_Vers by Holotech Enterprises http://www.holotech.net/";;
replace it with:
$ph_Vers = "";
$ph_HTLink = "";
$ph_HTTag = "";
