I don't see that either of them works, as there is no submit button.

This comment suggests to me that your new host has register_globals turned off 
in their PHP configuration. Check if that is the case. Phorm Jr. requires it 
to be on. I will change that eventually, but I don't anticipate working on Jr. 
in the near future. 

The full version of Phorm can work with register_globals off; if you're 
comfortable working with Jr., you should have no problem moving up to Phorm.
