I am experiencing this problem as well. I received a Phorm Alert email each 
time I tried to upload two files. It says: 

Internal Alerts: 
120 - Unable to open file upload module. Uploaded file(s) not saved. 

And further down it has:

PHORM_FILE02: /tmp/php2r7h2w 
PHORM_CMODE01: 2 
PHORM_CMODE02: 3

PHORM_FILE01_name: 000.gif

PHORM_FILE01_type: image/gif 
PHORM_FILE02_name: 019.gif 
PHORM_FILE02_size: 2775 
PHORM_FILE02_type: image/gif 
PHORM_NAME: WarbleSpec 

I don't know if this is a PHP config matter or a hosting service limitation or 
an improper Phorm setup.
