Hi there, 

For security reasons, phorm files must be inside the phorm installation 
directory, as all path information is stripped from the files you specify. For 
more information read the section on security in the documentation. 

If you are using PHP on a ack page, you must set 

$PHORM_PARSPHP = true; 

in your config file to allow the page to be parsed for PHP.
