I actually have the form working but I can't get rid of the error below. I get 
a page that says your information has been sent but on the top of the page 
you'll get the following message. Both the form email and acknowledge come 
through fine. 

Notice: Undefined variable: HTTP_HOST in E:\Webs\phorm.php on line 46 

this is line 46: if ($HTTP_HOST != "www.phorm.loc") error_reporting(E_ERROR | 
E_WARNING | E_PARSE); 

I've changed www.phorm.loc to everything I could think of, phorm.com, the host 
domain name, plus some others. I'm stumped. Although it works, I need to 
remove this error message.
