I'm part way through a similar migration right now. My EM 12.1.0.5.0
OMS is on OL5. The 11.2.0.3 repository DB was on the same server, but
I've upgraded it to 12.1, then migrated to an OL7 box that may someday
also host the EM13cR2 OMS. But that move requires an intermediate OL6
box, as EM12c won't work on OL7 and EM13cR2 won't work on OL5.
A good place to start for your migration is

to move the Repository (if needed) and
http://docs.oracle.com/cd/E24628_01/install.121/e24089/ha_backup_recover.htm#EMADV10754
to move the OMS. However, there's some confusion in those docs so I
opened an SR with Oracle Support anyway.
FWIW, from recent posts here about usability issues with EM13c, I've
suspended my upgrade for now. Check out the list archives for August at
https://www.freelists.org/archive/oracle-l/08-2017. Also, search the
older archives at
https://www.freelists.org/archive/oracle-l for "OMS
move" (sans quotes).
I'm not sure why this task needs to be so complex, but it is. Take your
time, plan it out, and test if you can.
