DBA_INDEX_USAGE

Hello all,
Please could somebody clarify for the me the distinction between these columns 
in DBA_INDEX_USAGE:
TOTAL_ACCESS_COUNT (Total number of times the index has been 
accessed)TOTAL_EXEC_COUNT (Total executions the index has participated in)
How can an index participate in an execution without being accessed?   Why are 
these numbers not the same?
