"Normal" or diagnostic B Scan is 76512 76510 implies diagnostic B scan pared with standardized A scan. Using this code is sure to trigger some pushback as it is not a commonly used combination. Putting the Vector A scan into the B scan is not the same thing. 76511 is for standardized A scan only, generally the code employed when doing oncology echography. Use 76512 for those 10MHz examinations of dense ocular media, retinal detachment, vitreous hemorrhage, etc. Be mindful that all CPT codes for echography are tightly linked to proper ICD.9 codes. You cannot just "do" a B scan and get paid for it. The most obvious and common use of B scan is for the dense cataract, pre IOL surgery. Alas, that is the trickiest Dx to use. You will not get paid for a routine screening B scan just because they have a cataract. You must justify a strong suspicion based on history, previous records, etc that something more ominous is suspected.
