[optimal] Education in the US Survey - Due September 13th

The goal of this short 5 minute survey is to identify how Ophthalmic
Photographers / Imagers in the United States initially got into the field.
The results of this survey will be presented at the International Conference
on Ophthalmic Photography in Oxford, England on September 22nd, 2011. After
ICOP, they will also be made available on the OPS Website. This survey will
be open until Tuesday, September 13th. Please do not take this survey if you
have never worked as an Ophthalmic Photographer / Imager in the United
States. Thank you in advance for your participation.
