We're using EnterpriseWizard.  It's not exactly simple, but it is very 
customizable and is cheap relative to Remedy 
(http://www.enterprisewizard.com/software-pricing.htm).  You might also look at 
RT (Request Tracker) which is a popular OSS ticketing system.

For smaller organizations - (200 users or so)

Are you aware of any relatively inexpensive, simple, easy to navigate Help Desk 
Software?  What is your organization using?
