Good day, NYSCHP members,
A member hospital would like to know how other NYS hospitals are handling blood
products as NYS DOH
regulations<https://regs.health.ny.gov/content/section-58-219-records-be-kept-when-derivatives-are-infused>
state when human blood derivatives (IVIG, Albumin, etc) are administered,
certain items must be documented in the patients chart or in records maintained
in the dispensing location (blood bank/pharmacy).
They include:
· Product name, manufacturer, lot number, and expiration date
· Documentation of visual inspection prior to infusion (clarity)
· Name of person administering the product
· Documentation of identification of the recipient
· Date of infusion and quantity of material infused
· Description of any adverse reaction and the results of related
investigation
In your facility, please respond A-E:
A. These items are exclusively documented by Nursing staff prior to/upon
administration; records are kept in the patient's chart (either paper or
electronically)
B. For blood-derived items dispensed by Pharmacy, Pharmacy documents some
of these items (e.g. lot number, expiration, clarity) but not all
C. Pharmacy documents some of these items for some items, but not all
(e.g. for albumin, but not for IVIG)
D. Pharmacy stocks some (e.g. albumin) in automated dispensing cabinets,
where some of the above information is captured upon removal (lot number,
expiration); the other required information is captured elsewhere (e.g.
patient's chart)
E. Other means of documentation (explain)
