Testimony: Blowout Preventer's History a Muddled Tale

The blowout preventer on BP's failed Macondo well had hydraulic leaks, was
modified without documentation and didn't comply with federal regulations,
but that doesn't mean it caused the deadly Deepwater Horizon disaster,
according to testimony in government hearing Wednesday.
The 450-ton stack of valves that sits on the sea floor above the well had
been modified repeatedly in the 10 years since it was built, partly with
help from manufacturer Cameron International, but mostly by Transocean
crews, said Mark Hay, one of Transocean's senior subsea engineers on the
rig.
But Harry Thierens, a BP vice president for drilling and completions
operations who was involved in efforts to shut the well down after it blew
out April 20, said his team didn't learn until May 1 that one of those
changes was confounding the effort to activate pipe rams designed to cut
and seal drill pipe in the event of a blowout.
Access ports were swapped for two sets of valves, meaning robot submarines
trying to activate a powerful set of pipe rams instead worked on a much
weaker set of test rams.
"I was quite frankly astonished that this could have happened," Thierens
told a joint investigation panel of the Coast Guard and Bureau of Ocean
Energy Management, Regulation and Enforcement, which is holding hearings
this week in Houston."When I heard this news, I lost all faith in this BOP
stack," Thierens said, using the industry shorthand for blowout preventer.
Problem persisted
But even after engineers discovered the problem, they couldn't get the
rams to close completely and stop the gusher. Subsequent work on the
damaged well indicates at least two sections of drilling pipe are lodged
in the blowout preventer. The pipe rams aren't designed to cut through two
pipes.
