I'd say the best option for formatting and "making it look pretty" is to
set up your questions in a table (each question consisting of a table of
2 columns and 5 rows). You could hide the gridlines and turn off
borders, and voila! Looks as though you're lined up perfectly without
any outside assistance!
Ain't tables grand?

What is the best way to do this? I tried numbering but could not get
