Hey Anne... There are a couple of ways to do this, depending on your skill level. You could simply wrap a bookmark around the initial info and then have that pulled into different locations, or you can use a simple inputbox to capture the info and pass that to each location, or you can create a custom dialog box and pass the info to each spot or even just use form fields.
