[mso] Table of Contents

I have some titles marked as header 2, and I just noticed that not all
of them appear in the table of contents. I did an "Update Field," then
"Update Entire Table" and some of the titles labeled Header 2 are not
appearing. I am using Office XP Word. Have I turned them all somewhere?
I am not sure how to troubleshoot this.
