[mso] Creating Forms using Excel

Can any one please help?
I am trying to create a Template in Excel to be used by Field Engineers so
that they can enter their daily work/time and send it to the office for
calculation and payment.
I am able to design the presentation but need help in the creation of the
form using the Control Toolbox, the respective Icons and the formatting of
these accordingly. Does anyone know of a good "Teach-yourself" website or
book.
