You use a service that makes the sender confirm they are the original sender of the e-mail before they'll pass it along to you. You can see how this would defeat most spammers who wish to remain anonymous and self-propagating viruses that don't have the capability to respond to a confirmation reply request. Of course, it's a pain for the mail sender... but I think senders that you OK (by way of the confirmation) get added to a "white list" of "friends", so that next time their mail goes on through.
