As an outside observer, did you try the most current version of Jaws?  Did
you encounter the same problem?

When we were running Windows98se, we used "EXTRA! Personal Client" version
6.5 to connect to our mainframe with JAWS 4.02 and then later with JAWS 4.5.
We were using version 6.3 before that even though it acted glitchy, but  the
mainframe OS got upgraded and then things went really weird for some reason.
We bumped to "EXTRA! Personal Client" version 6.5 and life was good again.
We did have to set cursor to underline and fast blink in the versions of
"EXTRA!" that gave us those options.  Version 6.4 never did play nice with
JAWS.  

Now we're on WindowsXP with (patched) JAWS 4.5 but had to switch from the
Attachmate products ("EXTRA!") to a different 3270 client because of cursor
tracking problems.  ("EXTRA!" version 6.5 was not certified or stable under
WindowsXP and versions 7.0, 7.01,  and 7.11 all seem to have cursor-tracking
problems with JAWS and ZoomText.)  I've been talking with Attachmate for the
last few months and they are trying to collaborate with developers from both
Freedom Scientific and AiSquared (the ZoomText people) to try to solve the
cursor tracking stuff for their newest version of "EXTRA!" (version 7.11 for
XP).   I hope they are all successful!

I worked almost two years ago with someone who has to use Extra for work.
Neither 6.3 nor 6.4 will work with Jaws.
I believe we used 6.6 but I don't remember for sure. You do have to change
the cursor and I believe someone already has described where to go in Extra
to change that.
