It's needed a lot more in Window-Eyes (it's a different keystroke there) than JAWS, but you use it if you have an application that uses the same key combination as JAWS. Let's say you were using a program that uses insert+t for something. Of course, if you hit insert+t, JAWS picks up the keystroke and reads the title bar to you. But if you hit the pass-through key first, then that insert+t would be sent to your application instead of to JAWS.
