Ok, in outlook you have several different ways to cause email to be sent
or received; are you choosing to trigger the process manually, and if
so, are you perhaps choosing to "receive only" instead of send/receive
all?

That might be one explanation for why it happens sometimes.

Another might be that you choose to send a message, then you immediately
close outlook before it has had time to complete the sending of the
message (you can tell what's going on by checking the status line with
the jaws command insert plus pagedown).
