Filling out a form in .pdf format

I have a consent form  in .pdf for the flu shot to fill out and it is difficult 
to read much less to find the check boxes and edit boxes.  

Can someone give me suggestions?  I check the JFW help information and couldn't 
find anything helpful. 

I have AdobeReader version 7 and I have tried JFW version 7 and version 8.

Any help will be appreciated.
