Anyone could have seen that the coding style of that particular part of PhotoFiler is unusual indicating that its author had a badly cooked egg (runny instead of hard boiled) for his breakfast on the day he wrote it, spilled egg-yolk on his shirt and hence was too annoyed to look up the PRM's description of that system call properly, thus introducing the bug that caused problems with the system upgrade. Of course, the true Holmes would have seen all that before the RISC OS 5.18 release.
