The internal host has to be a secureNAT client.
Take a look at www.isaserver.org/pages/learning%20zone.htm, and specifically at:

  Hi all,

Im pretty new to ISA, and I am trying to configure NAT for my client PC's so
they can. well for starters, just ping an external IP.

I can't seem to get it going.

Anyone point me in the right direction?

  Ta.
