You only need to create a protocol rule 4899 and set your client to SecureNAT 
concept. It's working fine to me.

Does anyone use Radmin through ISA to external clients?

I have opened the 4899 TCP ports to allow access in and out but we seem
to be unable to connect to the clients server still. Our config is
internet - router on external IP - ISA with external + internal NIC's -
internal network.
Is it the router or ISA external address that is required to be given to
the host server to allow us access?
