This might be problem with Browser's Keep Alive Connection settings
(i.e. tcp/ip connection is broken after 1+n request is issued).
If u r  using IE Try to change HTTP connection settings in IE config
if not switch to IE.

After installation of ISA server, users were able to access the web
without any problem but some images on each site visited were coming up
with red(x)s where the images should be.  When the page is refreshed
some
images appear but still alot missing.

Not we are using satellite connection and bandwidth is not a problem,
nor
the site because when the you right click on the (x)s the images appear.
