Start the ISA server in safe mode and delete the cache. Reboot and it will be fine. I also found that by reinstalling the ISA patches in the correct order resolved this problem permanently. I could not use the "delete cache" tool from the ISA tools site as the server was unstable after the rebooting. Rebooting in safe mode was the only solution that worked for me.
