Here's what we do, cost = 0$ also. Run a reporting tool, we use Loggerythm (basically because I wrote it :) at http://www.loggerythm.com/ for the moment this a free, this makes nice HTML reports on who is doing what, we just simply make these public (within the organisation), if someone wants to surf porn then they have to live with the fact that everyone will know about it (and they are all aware of this), this works very well, maybe not so good for schools :) but certainly for other places where the work force is adult and have been warned and should know better, I just had a look down the top 100 sites surfed here and not a one is porn site, that's for about 250 users, this is a lot cleaner than it was when it was all hidden. Like I said, this solution is not for everyone but it does have the advantage of not getting into blocking stuff which is something I don't really want to do if I can get out of it. I only block a few MP3 sites but that's a bandwidth issue and not a censorship issue.
