IMS Queues

E5.5 question for you all.
 
I am running exchange 5.5 SP4 on win2k SP2.  I constantly get a lot of weird
messages in my "outbound messages awaiting delivery que" almost every day I
had to manually delete them.
 
My exchange server is internal with a unix sendmail server acting as a
xternal SMTP server which relays email from the outside to my exchange.  
 
Where is the flaw that is letting all this spam thru?  
 
I am using Trend Micro Scan Mail to help with the spam.  it does a good job,
but i still get this anoying email in my queus.
 
Can anyone please help.
