Thanks to all for your comments, but I would think that this rule for underlining the heading is to keep it separate from the items in the column. The heading should be distinguished from them in some way. The only other way would be to center the heading. But with a long heading and short items, centering might not make it obvious or with a single column across the page that is more like just a list, having a short centered heading, it might not be as easily found as if it were left justified with a line after it.
