I once asked an Amish trainer I knew how he got his horses to stand so well.

"Get in and drive 5 miles."  Eventually they will learn that they don't 
know how long they are going to be working, so they will welcome any 
chance to stand still."

I also remember hearing Udo Hochgeschurz explain that his son rode his 
pair daily, and then on weekends they would hitch and for conditioning 
drive for 4 hours or so in the National Forest, taking along the 
newspaper and coffee.
