I had a similar problem once.  It is possible that the submitter had
to insert page numbers and just got off in the numbering. That might be the
problem, and would explain a seamless transition from page to page.

Thanks, that would be great. Funny thing is that I didn't even realize that
the page was missing.  It blends seemlessly to me, but that was one of the
reasons why it wasn't aproved.
