The author has a claim for the Intro, Appendix, and Bibliography for 2000, and according to that record there is a previous registration for 1998, but I didn't find the record for that edition, so I don't know if she holds the copyright for the body of the book or not. If the copy on Bookshare is the later edition, then according to what I read on the website, the copyright would be Leslie Cannold 2000, but it all depends on which edition you have.
