GP,

When you say the book is flat on the scanner, are you
exerting pressure on it or just laying it flat,
perhaps with the cover on it? If the latter, that
isn't enough, as I found out when I was new.
Apparently the cover is just a dust cover. One has to
press on the book, snas cover, especially at the
binding, getting the pages near the binding as close
to the platen as possible. Sometimes, if the book is
wide and one's hands aren't large, while pressing the
binding the other edge of the book isn't getting
enough pressure so the junk characters occur. The only
solution I know of is to have hands that are large
enough to apply equal pressure to binding, middle of
book and other edge of book. grin.
