It sounds to me like the OCR program took it as two columns, the one with
the words, and the one with the numbers, and processed it that way.
So now the numbers don't match up to the chapter headings.
Table of Contents should be processed as a table and then converted to text,
or as one single column and then sometimes you have to put in a paragraph
return after the page number.
