atw: TOC from PDF - Directly

Does anyone know of a tool or process I can use to generate a TOC for a
200-page Product List that only exists as a PDF? (The Product List has
never existed as, say, a FrameMaker or Word document. It's created
directly from Baan/Brio ERP software, which can only output PDF or
HTML.)
I can use software such as ARTS PDF Aerialist to generate PDF bookmarks
from heading text styles, but creating a Table of Contents automatically
. . . hmmmmm.
