Nigel.
If you're documenting for a Windows based audience, they're already =
familiar
with Microsoft's documentation conventions.
For example, the MSDN article on 'Shortcut Keys in Menu Items' states:
"... Match key names to those inscribed on the key caps. For example,
display CTRL and SHIFT key combinations as CTRL+key (rather than =
CONTROL+key
or control+key or ^+key) and SHIFT+key. When using function keys for =
menu
item shortcuts, display the name of the key as Fn, where n is the =
function
key number. ..."
As you can see, they don't use angle brackets or spaces.

Is there a particular convention to use?
I've seen similar to the following:
<CTRL> + <ALT> + H.
