I created forms for my parents business many years ago and kept modifying it
as years went on. (I'm the computer guy in the family.) Finally got it to
where they wanted it about 5 years ago and I use the same form for my
business too now.
>From memory, the main things are:
thread colors
backing
topping
placement of embroidery
order date
receive date
who ordered from
date delivered/shipped
and of course the item style number, colors and sizes

There are more but I do not have a form with me right now.
