I'll assume you are writing for a UK audience.

The order is typically, but by no means universally:
Contact details
Education
Employment
Skills
Interests
References.

You can swap the skills and employment sections if one is more heavily
weighted than the other.

Dates don't need to be precise, giving months or year timeframes is fine.

Remember formatting is important - you can tweak things to make it look
better on the page.  Avoid too much white space, refrain from tables or
anything else that draws attention away from the key sections.  Try not to
break sections across pages.

Finally, as an offer, if you'd like me to look over anything for comment,
I'm happy to do this.  I have recent experience doing this both for myself
and others, so if you need a friendly (or critical if you wish) eye to look
over it, just shout.
