Some tidying up on this.
The CLI can now formally add, and swap instances, and they will be correctly
numbered. They can't yet be deleted.
Although in theory LV2 still has no limit on instances, for stand-alone I've
restricted both the CLI and GUI to 32 (0 - 31), although the user IDs can be
whatever you like. I think it will be avery long time before anyone will have a
computer that can realistically handle more!
There are still some oddities with instance start-ups and defaults which need
cleaning up, but as far as I can tell, nothing dangerous.
