Yadex bug fixes

These patches fix serious problems in Yadex.
Several patches are in isolated files, but all the fixes as I am using them
now are in yadex_patch05.patch.
I could spend time trying to isolate these into separate patches, but have
not got around to it, and I have been holding on to them too long.
Another person on the FreeDoom project needed them, so it is time to get
them distributed. I use Yadex for all my FreeDoom work.
Please release an updated version of yadex with these patches.
I have other fixes planned.
