We had this some time ago(years) so I'm going to be a bit vague. There
is a file called something like iconcache.dll - this may have become
corrupted. Have a search on MS KB on corrupt icons; there is an article
relating to this.

Subject: [windows2000] All icons on start menu are now black squares

I have one user who says she didn't do anything but now all the icons in
her Start Menu are black squares. All the icons in the quick start menu
are also black squares.

I uninstalled and reinstalled Outlook Express and that icon is back. I
selected properties for IE in the quick start area and selected change
icon, and selected the original icon and that one is back also.

Any idea why that happened and is there a way to get them back without
reinstalling (or repairing) each program and windows itself?
