Are these devices using the USB class driver?  What form factor do they show up 
as (speakers, microphone, headset, etc.)?

Thanks for the quick answer, but that is not what we observed.  We were 
plugging devices with cpl off.  After plugin the cpl reliably shows original 
device as being default.  We tried to plug in 3 different types audio devices 
which were supposed to switch the default with the same result.  WMP reliably 
plays into original default device and all system sounds for example volume 
slider tone goes to the new device.  We reinstalled the image several times and 
result was consistent.  Obviously something is screwed up in the image, but I 
have no idea where to look at.

Is mmsys.cpl running when you plug the USB device in?  Mmsys.cpl should 
indicate the default change immediately, but I do have a bug report that says 
it sometimes (very rarely) doesn't.  Restarting mmsys.cpl should fix it.

For WMP to use the new device you need to restart WMP, or just play a new song. 
 E.g. if you are listening to an album, it will use the new default when the 
next song starts.

We have a Vista platform where audio system behaves quite funny.  When you plug 
in a device which supposed to become a default playback (for ex. USB speaker), 
all system sounds are directed to the new device.  However control panel still 
shows previous device as the default, and output from the media player is also 
directed to the previous default device.   Any suggestions on how to debug this 
would be appreciated.
