I have experience with Google forms and survey monkey. Both seem to be
accessible with screen readers and are free as far as I know.

less. Currently, the top two contenders are Wufoo and JotForm. If you’ve ever
used these tools, particularly from an administrative perspective, and can
comment on how accessible/easy to use they were, I’d be very interested in
hearing about your experience. If you know of another product that’s worked
well (or not worked well) for you, I’d be interested in hearing about it, too.
