1. Errors have crept into the reports.
2. These reports are tabulated by computer
and are DUE on the 1st of the following month
and must be RECEIVED ON or BEFORE the 5th.
3. Use only the standard 16 line format.
4. Use only your personal callsign in the
FM line.
5. There are no colons in the header.
6. Do not use descriptors in the items.
7. Items 9 through 12 are ONLY filled in
if changed.
8. The place for remarks is in item 13.
