NYSARA Issues Support Memo Protecting Retiree Health Insurance Premiums

NYSARA is  concerned about the proposals in Governor Cuomo’s Executive Budget 
which once again target fixed-income retirees.  The governor’s plan to cap and 
eliminate certain Medicare Part B reimbursements and increase health insurance 
costs is unfair and bad policy.  These proposals, which seem to appear in one 
form or another every year since 2016, must be rejected.

The governor’s proposal would cap reimbursement of the Medicare Part B standard 
premium at the current levels and cease reimbursement for the Income Related 
Monthly Adjustment Amounts (IRMAA) for higher-income state retirees.  
Specifically, the proposal would cap the standard premium reimbursement at 
$148.50 per month for retirees and their eligible dependents.  Any future 
increases would be subject to budget negotiations.  In addition, the state 
would retroactively discontinue the IRMAA Medicare Part B reimbursement back to 
January 1, 2021.  The new IRMAA burden alone will cost affected retirees 
anywhere from $713 to $4,277 annually.

The governor has also proposed a sliding scale for retiree health insurance 
costs for new employees, creating yet another barrier for recruitment and 
retention of state workers at a time where that recruitment is critical.  Any 
future employees who work less than 30 years for the state would pay a 
proportionally higher percentage of health care costs in retirement.

Capping and eliminating these insurance reimbursements and increasing retiree 
health care costs will greatly impact retirees who dedicated their lives to 
public service and are now on fixed incomes.  Public employee retirees do not 
have the financial flexibility to go out and earn extra money in response to 
budgetary whims.  It is patently unfair to pick on these individuals and ask 
them to fill budget shortfalls long after they have left the workforce 
especially because this represents a bait and switch policy.  When retirees 
make a decision to retire they take all their retirement costs into 
consideration.  To ask them to spend hundreds or even thousands of dollars a 
month more that they had not anticipated at retirement is both unfair  and 
unnecessary. Balancing the state budget should not be a burden they have to 
bear. NYSARA members must ask their legislators to remove Parts CC, DD and EE 
of the Public Protection and General Government Article VII Budget Bill 
(A.3005/S.2505) to reject these bad proposals that would increase health care 
costs for state retirees.

Medicare Reminder Telehealth Services

A telehealth service is a full visit with a provider using telephone or video 
technology that allows for both audio and video communication. Original 
Medicare covers certain telehealth benefits, such as professional 
consultations, office visits, including psychiatry services, and a limited 
number of other services.

Original Medicare only covers telehealth in limited situations:

  *   You live in a rural area and travel from your home to a local medical 
facility to get telehealth services.
  *   You require telehealth services to treat behavioral health conditions, 
including substance use disorder. You have the option of accessing telehealth 
services from your home or from a medical facility.
  *   You require telehealth services to diagnose, evaluate, or treat symptoms 
of acute stroke. You have the option of accessing telehealth services from your 
home or from a medical facility.

Medicare Advantage Plans must cover all of the telehealth benefits included in 
Original Medicare, but they may also cover additional telehealth benefits. 
Additional telehealth benefits offered by Medicare Advantage Plans may be 
available if you live outside of a rural area and may be covered in your home. 
If you have a Medicare Advantage Plan, you should contact your plan to learn 
about its costs and coverage rules for additional telehealth benefits.

Legislators in Hawaii, Maine and Washington recently introduced bills that 
would impose an 80% tax on drug price increases that are not supported by 
evidence of improved clinical value. Oklahoma and Rhode Island lawmakers have 
filed bills that would set the rates paid for up to 250 of the costliest drugs 
to rates paid by the four most populous Canadian provinces, which they say 
could reduce prices by an average of 75%.



Experts and the American public agree that the problem is growing. According to

averaged 3.44 times higher.



“We need Congress to pass legislation that allows Medicare to negotiate lower 
prices,” said Richard Fiesta, Executive Director of the Alliance. “The action 
at the state level is necessary because the Senate did not act on H.R. 3, 
legislation passed by the House last year. We must finally hold drug 
corporations accountable for making medicines affordable for everyone who needs 
them.”

House Committee Will Focus on USPS Next Week

After months of continued Postal Service delays, the House Committee on 
Oversight and Reform will hold a hearing on the Postal Service on February 24. 
Among the witnesses are Postmaster General Louis DeJoy, American Postal Workers 
Union President Mark Dimondstein and Postal Service Inspector General Tammy L. 
Whitcomb.



Committee Chairwoman Carolyn Maloney says, “The hearing will examine 
legislative proposals to place the Postal Service on a more sustainable 
financial footing going forward while preserving the delivery performance 
standards on which the American people rely.”



Older Americans rely on the Postal Service to deliver prescription drugs and 
other essential goods, and increasing numbers of Americans are reporting delays 
in delivery of their bills, resulting in substantial late fees and penalties.



The delays are a result of so-called “reforms” implemented by Mr. DeJoy, 
including elimination of overtime and suspension of requirements for timely 
delivery. Postal service employees have also been hit hard by the pandemic, 
with thousands of workers unable to work due to illness or quarantine 
requirements.



“Everyone relies on the post office,” said Joseph Peters, Jr., 
Secretary-Treasurer of the Alliance. “Americans have relied on it for 200 years

heart failure. Worry over whether she’d bring covid-19 to her father has been 
on Davidson’s mind daily. But recently he was vaccinated ― “an indescribable 
relief,” she says. (Kevin Parker)

Before her stretched a line of people waiting to get covid-19 vaccines. “It was 
agonizing to know that I couldn’t get in that line,” said Davidson, 50, who is 
devoted to her father and usually cares for him full time. “If I get sick, what 
would happen to him?”


Tens of thousands of middle-aged sons and daughters caring for older relatives 
with serious ailments but too young to qualify for a vaccine

themselves are similarly terrified of becoming ill and wondering when they can 
get protected against the coronavirus.



Like aides and other workers in nursing homes, these family caregivers 
routinely administer medications, monitor blood pressure, cook, clean and help 
relatives wash, get dressed and use the toilet, among many other 
responsibilities. But they do so in apartments and houses, not in long-term 
care institutions — and they’re not paid.



“In all but name, they’re essential health care workers, taking care of 
patients who are very sick, many of whom are completely reliant upon them, some 
of whom are dying,” said Katherine Ornstein, a caregiving expert and associate 
professor of geriatrics and palliative medicine at the Mount Sinai medical 
school in New York City. “Yet, we don’t recognize or support them as such, and 
that’s a tragedy.”
