When I set up the servers I installed all of the available hotfixes. Going forward I install them on an as-needed basis. I have not encountered any issues with bad interactions at the farm level. Our W2k box has all the hotfixes up through November of last year and our W2k3 boxes have all the hotfixes through August of this year. One word of caution - I would not recommend publishing desktops between the two platforms.
