I had this happen when of my users traveled to one our remote sites. He
worked there for a few days and when he left he neglected to logoff the
workstation he was working on. When he returned to the main office
coincidentally he had to change his password as per GPO. When he changed
the password at the main office the machine that he was still logged on
to at the remote office was sitting there with his old credentials, thus
creating a lockout.

Maybe ask your user if they've been logged on to other machines
recently.

Subject: [THIN] Urgent Help! User logging into Citrix farm is locked out
Importance: High

I have a user who logs into Citrix (2 server load balanced XPa, FR3,
SP3, Win2k SP4) and they are able to logon. As soon as they click on
Outlook they are prompted to logon to Outlook.

1) I have investigated and found this only happens to the user when they
logon to one of the 2 Citrix servers.

2) As soon as the user logs into Citrix the account is locked out.

3) This is not happening to everyone...so far only one person.

4) I am running lockoutstatus.exe but it really is not giving me more
info.

5) I tried upping the lockout from 3 to 6 but still the same.

6) I have checked replication monitor and the domain seems fine.

7) I restarted the operations master DC.

8) I have tried recreating profiles with and without terminal services
roaming profiles.

Anyone have any ideas?????? I need this working ASAP and I'm lost for
further ideas...
