We had this problem and found that we had installed the Exchange admin tools, which use a different MAPI32.dll, onto servers that were running Outlook. Removing the tools fixed our problems. "A recently installed program may cause Microsoft Office or other e-mail-enabled programs to function improperly. Outlook can resolve this conflict without affecting the program that originally caused the problem. Do you want Outlook to resolve this problem?" The workaround here http://support.microsoft.com/default.aspx?scid=kb;EN-US;256946 seems to have fixed that problem.
